How it works

Salicylic acid acts as a keratolytic, softening and removing thickened keratin, while zinc oxide provides a mild astringent and protective barrier action that soothes and shields the surrounding skin.

Prescribing in practice

  • Confine application to the affected lesion and protect adjacent healthy skin, as salicylic acid can cause irritation and erosion of normal skin.
  • Avoid use on the face, anogenital region, broken or inflamed skin, and on lesions in patients with diabetes or peripheral arterial disease without specialist advice.
  • The protective zinc oxide base may stain clothing and form an occlusive layer over the treated area.

Monitoring

No laboratory monitoring is required; review the treated lesion for response and excessive local irritation.

Counselling the patient

  • Pare or file down softened skin between applications and apply the paste to the lesion only.
  • Continue regular use over several weeks as directed.
  • Discontinue and seek advice if marked soreness, redness or skin breakdown occurs.

Evidence & guidelines

Salicylic acid is the established keratolytic active, with zinc oxide long used as a soothing, protective excipient in dermatological pastes.
