SORT (Surgical Outcome Risk Tool)

6-variable perioperative mortality risk tool developed in UK. Predicts 30-day mortality after inpatient surgery. Validated across 100,000+ surgical cases (NELA database). More parsimonious than POSSUM.

Score interpretation

Low Risk 0–2

SORT 0–2 — predicted 30-day mortality <1%

→ Standard perioperative care; elective post-op ward management appropriate; consent reflects low risk

Moderate Risk 3–5

SORT 3–5 — predicted 30-day mortality 1–5%

→ Anaesthetic review; HDU post-op planning; risk discussion with patient; document predicted mortality; optimise reversible risk factors; consider CPEX

High Risk 6–11

SORT ≥6 — predicted 30-day mortality >5% (potentially >10–20%)

→ Senior consultant involvement; full MDT discussion; ICU planning; documented consent with estimated mortality; consider alternatives to surgery; perioperative specialist input (e.g. CPET, haematology for blood product planning)
