Brugada Phenocopy Score

Distinguishes true Brugada syndrome from Brugada phenocopy (BrP) — transient Brugada ECG pattern due to reversible conditions. High score = true Brugada syndrome.

Score interpretation

Brugada Phenocopy Likely 0–1

Score 0–1. Brugada phenocopy more likely. Look for reversible cause.

→ Treat precipitating cause (fever, drugs, electrolytes). Repeat ECG after cause resolved. Monitor for resolution.

Indeterminate 2–3

Score 2–3. Cannot exclude true Brugada syndrome.

→ Refer to inherited cardiac conditions (ICC) clinic. Consider sodium channel blocker provocation test (ajmaline/flecainide). Genetic testing.

True Brugada Syndrome — High Probability 4–5

Score 4–5. High probability of true Brugada syndrome.

→ Cardiology/ICC clinic referral. Risk stratification for SCD (symptomatic = high risk). Consider ICD. Genetic counselling. Avoid contraindicated drugs (see www.brugadadrugs.org).

Interpretation bands for the Brugada Phenocopy. Apply clinical judgement and local guidance.
