Score interpretation

Low Bleeding Risk 0–1

~2.5% annual major bleeding risk

→ Anticoagulation generally appropriate if CHA₂DS₂-VASc ≥2 (men) or ≥3 (women)

Moderate Bleeding Risk 2–3

~5–6% annual major bleeding risk

→ Weigh against stroke risk; DOAC with lowest bleeding profile; frequent monitoring

High Bleeding Risk 4–12

>10% annual major bleeding risk

→ Consider withholding anticoagulation if risk exceeds benefit; modify correctable risk factors; haematology review
