Fisher Grade (SAH)
Grades amount of subarachnoid blood on CT to predict vasospasm risk
Score interpretation
Grade 1–2: Low risk of symptomatic vasospasm (~21–25%)
→ Standard SAH management; nimodipine; serial TCD/neurological monitoring
Grade 3–4: High risk of symptomatic vasospasm (~33–37%)
→ ICU monitoring; early aneurysm treatment; aggressive vasospasm prevention protocol; daily TCD
Interpretation bands for the Fisher Grade. Apply clinical judgement and local guidance.
