FeverPAIN Score (Streptococcal Tonsillitis)
Predicts likelihood of Group A streptococcal pharyngitis/tonsillitis to guide antibiotic prescribing. Developed in UK primary care.
Score interpretation
Low likelihood of streptococcal infection (13-18%)
→ No antibiotic; symptomatic treatment (paracetamol, ibuprofen, fluids); safety net
Intermediate likelihood (~34-40%)
→ Back-up (delayed) antibiotic prescription; review in 3-5 days; RADT if available
High likelihood (62-65%)
→ Immediate antibiotics: phenoxymethylpenicillin 500mg BD 5 days (or amoxicillin 500mg TDS 5 days); clarithromycin if penicillin allergic
