Score interpretation
Full function — independent 6
→ Promote continued independence. Reassess at clinical change.
Moderate impairment 4–5
→ Identify supportable domains; OT review, equipment provision, carer training. CGA indicated.
Severe functional impairment 0–3
→ Care package essential — domiciliary or residential care. Capacity assessment, advance care planning, carer support. Address reversible contributors (pain, polypharmacy, sensory loss, depression).
