→ Consider liver biopsy or FibroScan; hepatology review; possibly treat if fibrosis F2+
Treatment indicated
→ Start antiviral: tenofovir (TDF/TAF) or entecavir; monitor HBV DNA and ALT 3-6 monthly; assess for HCC risk
