Lauge-Hansen Classification (Ankle Fractures)

Classifies ankle fractures by mechanism (supination vs pronation) and stage of ligament/bone injury. Guides surgical vs conservative management.

Score interpretation

Weber A / Isolated lateral fracture

→ Conservative: CAM boot or below-knee cast; weight-bearing as tolerated; no surgery needed if stable

Weber B (stable — no medial injury)

→ Conservative management; reassess with stress views if unstable

Weber B (unstable — bimalleolar)

→ ORIF fibula + medial; plate and screw fixation; assess syndesmosis

Weber C / Trimalleolar / Syndesmosis injury

→ ORIF: fibula fixation + medial repair + posterior if >25%; syndesmosis screw; 6 weeks non-weight-bearing
