SCOFF Questionnaire for Eating Disorders
5-item screening questionnaire for anorexia nervosa and bulimia nervosa. Validated in primary care. Score ≥2 = likely eating disorder. SCOFF = Sick, Control, One stone, Fat, Food.
Score interpretation
SCOFF 0–1 — eating disorder unlikely
→ No immediate eating disorder intervention; maintain awareness; offer healthy eating advice; reassess if clinical concerns arise
SCOFF ≥2 — possible eating disorder
→ Full eating disorder assessment; BMI, vital signs, electrolytes (K, phosphate, glucose), ECG (QTc); refer to eating disorders service; MARSIPAN guidelines if medically unstable; involve dietetics; inpatient admission if BMI <13 or medical complications; involve family/carers
