Interferon gamma-1b is a recombinant immunomodulatory protein given by subcutaneous injection, used to reduce the frequency of serious infections in chronic granulomatous disease and in severe malignant osteopetrosis.

How it works
It augments phagocyte oxidative metabolism and macrophage function, enhancing the killing of intracellular pathogens.
Prescribing in practice
- Flu-like symptoms (fever, chills, myalgia, headache) are very common, especially early in treatment, and can be mitigated by bedtime dosing and antipyretics.
- Use with caution in patients with pre-existing cardiac disease or seizure disorders, which may be exacerbated.
- It can cause reversible neutropenia and hepatic enzyme elevation, particularly in younger children.
Monitoring
Monitor full blood count, renal and liver function periodically during therapy.
Counselling the patient
- Flu-like effects are common at first and often ease with continued treatment.
- Use the injection technique you have been taught and rotate injection sites.
- Store the product refrigerated and do not freeze or shake it.
Evidence & guidelines
Use in chronic granulomatous disease is supported by a landmark placebo-controlled trial demonstrating reduced serious infections (International CGD Cooperative Study).
