Netilmicin

Netilmicin is an aminoglycoside antibacterial used parenterally for serious Gram-negative infections, including some resistant to other aminoglycosides.

How it works

It binds to the bacterial 30S ribosomal subunit, causing misreading of the genetic code and inhibiting protein synthesis, giving a concentration-dependent bactericidal effect.

Prescribing in practice

  • Like all aminoglycosides it carries a dose- and duration-related risk of ototoxicity and nephrotoxicity, so therapeutic drug monitoring and renal function checks are essential.
  • Risk is increased in renal impairment, the elderly, with prolonged courses and with concurrent nephrotoxic or ototoxic drugs.
  • It should be avoided in myasthenia gravis as aminoglycosides can impair neuromuscular transmission.

Monitoring

Monitor serum drug concentrations, renal function and auditory/vestibular function during treatment, especially with longer courses.

Counselling the patient

  • Report any hearing loss, ringing in the ears, dizziness or reduced urine output.
  • Tell the team about any kidney problems or other medicines you take.
