LABA Pregnancy: There are no data from use in pregnant women. Animal studies do not indicate direct or indirect harmful effects with respect to reproductive toxicity at clinically relevant exposures. As a precautionary measure it is preferable to avoid use during pregnancy. Like other beta2-adrenergic agonists, olodaterol may inhibit labour due to a relaxant effect on uterine smooth muscle. Breast-feeding: no clinical data in nursing women and it is unknown whether olodaterol/metabolites are excreted in human milk (excretion shown in animals); since systemic exposure is negligible at the human dose of 5 micrograms per day, relevant effects on the breastfed infant are not expected — decide whether to discontinue breast-feeding or therapy (§4.6).

Olodaterol is an inhaled long-acting beta-2 agonist (LABA) used as maintenance bronchodilator therapy in chronic obstructive pulmonary disease.

As a once-daily maintenance bronchodilator it should not be used for the treatment of acute episodes of bronchospasm, i.e. as rescue therapy. Paradoxical bronchospasm may occur and may be life-threatening — discontinue immediately and substitute alternative therapy. Administer with caution in cardiovascular disorders (especially ischaemic heart disease, severe cardiac decompensation, cardiac arrhythmias, hypertrophic obstructive cardiomyopathy, hypertension, aneurysm), convulsive disorders, thyrotoxicosis, known or suspected QT prolongation (e.g. QT >0.44 s) and in patients unusually responsive to sympathomimetic amines; experience is limited in patients with MI in the previous year, unstable or life-threatening arrhythmia, hospitalisation for heart failure in the previous year, or paroxysmal tachycardia >100 bpm. Beta2-agonists may produce significant hypokalaemia (potentiated by hypoxia and concomitant treatment in severe COPD) and inhalation of high doses may increase plasma glucose. Caution with planned operations using halogenated hydrocarbon anaesthetics due to increased susceptibility to adverse cardiac effects. DEVICE: the inhaler is for a single patient; the re-usable inhaler can be used with up to 6 cartridges and should not be used for more than one year after the first cartridge is inserted. Each prepared inhaler delivers 60 actuations (30 doses). If not used for more than 7 days, release one actuation towards the ground; if not used for more than 21 days, re-prime until a cloud is visible then repeat three more times. Dose adjustments

Renal

Renally impaired patients can use Striverdi Respimat at the recommended dose. There is limited experience with its use in patients with severe renal impairment (§4.2).

How it works

It selectively stimulates beta-2 adrenoceptors on airway smooth muscle, causing sustained relaxation and bronchodilation.

Prescribing in practice

  • It is a long-acting maintenance bronchodilator and must not be used for relief of acute bronchospasm or as monotherapy in asthma.
  • Use with caution in cardiovascular disease and conditions sensitive to beta-agonists, and be aware of the risk of hypokalaemia.
  • It is frequently prescribed in a fixed-dose combination inhaler with a long-acting antimuscarinic for COPD.

Monitoring

Monitor symptom control, lung function and exacerbation frequency, and remain alert to tachycardia, tremor and hypokalaemia.

Counselling the patient

  • Use this inhaler regularly to keep your airways open, not for sudden breathlessness.
  • Keep a separate fast-acting reliever inhaler for emergencies.
  • Report palpitations, marked tremor or muscle cramps.
