Recognise + Risk Period
Suicide is leading direct cause of maternal death (MBRRACE-UK). Key periods: pregnancy + 12 months postpartum.
Conditions to recognise:
• Postnatal blues — common, transient, days only.
• Postnatal depression — up to 1 in 10.
• Postpartum psychosis — 1–2 per 1000; emergency; rapid onset within 2 weeks postpartum.
• Bipolar disorder — high relapse risk in pregnancy / postpartum.
• Anxiety disorders, OCD, PTSD (post-traumatic birth).
• Eating disorders.
• Substance / alcohol use.
Screen all women: Whooley + GAD-2 questions at booking + 4–6 weeks + 3–4 months postnatal.
