GRACE Score (ACS Risk)

Global Registry of Acute Coronary Events (GRACE) score predicts in-hospital and 6-month mortality in ACS. Guides invasive vs conservative strategy.

Used in: Acute Coronary Syndrome & Chest Pain

Score interpretation

Low risk (GRACE < 109): In-hospital mortality < 1%

→ Conservative or early invasive strategy; discharge within 72h with outpatient follow-up

Intermediate risk (109-140): Mortality 1-3%

→ Early invasive strategy preferred; angiography within 72h; DAPT + anticoagulation

High risk (GRACE > 140): Mortality > 3%

→ Urgent invasive strategy; angiography within 24h; CCU admission; optimise medical therapy
