INTERCHEST Clinical Prediction Rule

Predicts probability of cardiac chest pain in primary care. Score ≥1 suggests cardiac cause.

Score interpretation

Low Probability — Non-Cardiac -1–0

INTERCHEST < 1: Low probability of cardiac chest pain. Musculoskeletal or other cause likely.

→ Reassure patient. Treat underlying non-cardiac cause.

High Probability — Cardiac 1–6

INTERCHEST ≥ 1: High probability of cardiac chest pain. Further cardiac evaluation warranted.

→ ECG, troponin, cardiology referral. Consider stress testing.
