LV Diastolic Dysfunction Assessment (ASE/EACVI 2016)

2016 ASE/EACVI algorithm for patients with normal LVEF: four variables (mitral annular e', average E/e', left atrial volume index, peak TR velocity) classify diastolic function as normal, indeterminate or dysfunctional. Grading and filling-pressure estimation need mitral E velocity and E/A ratio.

Average of septal and lateral E/e' (abnormal > 14).

Abnormal > 34 mL/m².

Abnormal > 2.8 m/s. Enter 0 if no measurable TR jet — the remaining criteria are then used.

Score interpretation

Normal Diastolic Function 0

Fewer than half of the available criteria are abnormal (ASE/EACVI 2016, normal LVEF).

→ If symptomatic with preserved EF, consider other causes of breathlessness; a diastolic stress test can help if HFpEF is still suspected.

Indeterminate 1

Exactly half of the available criteria are abnormal — diastolic function cannot be classified.

→ Use additional parameters (e.g. Valsalva, pulmonary vein flow, LA strain) or a diastolic stress test; clinical context essential.

Diastolic Dysfunction Present 2

More than half of the available criteria are abnormal (ASE/EACVI 2016, normal LVEF). The grade and LV filling pressure need mitral E velocity and E/A ratio (Algorithm B), which this calculator does not collect.

→ Grade with the full ASE/EACVI algorithm; treat underlying hypertension, AF and other drivers; consider natriuretic peptides and cardiology review if HFpEF is suspected.
