Score interpretation

Very good baseline survival 1

→ Adjuvant therapies offer marginal absolute benefit. Endocrine alone often sufficient (ER+). Discuss patient preference re: chemotherapy in low-risk disease.

Good baseline survival 2

→ Endocrine ± targeted therapy. Chemotherapy adds modest absolute benefit — Oncotype DX / EndoPredict to refine in ER+/HER2−/N0.

Moderate baseline survival 3

→ Multi-modal adjuvant therapy adds meaningful absolute benefit. Discuss chemo + endocrine + HER2-targeted as appropriate. Consider clinical trial.

Poor baseline survival 4

→ Aggressive multi-agent chemotherapy + targeted therapy + bisphosphonate (zoledronate per AZURE). BRCA testing for olaparib eligibility (OlympiA). Trial enrolment.
