Kruis Score for Irritable Bowel Syndrome

Scoring system to diagnose Irritable Bowel Syndrome and distinguish it from organic bowel disease. Combines symptom scores with laboratory findings. Score >44 supports IBS diagnosis.

Score interpretation

IBS Likely

→ Kruis >44: High probability of IBS. Organic disease unlikely. Positive diagnosis of IBS: lifestyle advice, low-FODMAP diet, antispasmodics (mebeverine, hyoscine), gut-directed psychotherapy.

Organic Disease Not Excluded

→ Kruis ≤44: IBS cannot be reliably diagnosed — organic disease not excluded. Investigate: FBC, ESR/CRP, coeliac serology, stool calprotectin, thyroid function. Consider colonoscopy, especially if alarm features present.
