Lower weight range — doses calculated accordingly
→ Always verify with weight-based dosing chart; adjust per aPTT at 6 hours
Standard weight-based heparin dosing
→ Monitor aPTT 6 hours after initiation and after each dose change. Target aPTT 60–100 seconds (VTE) or per local protocol
High calculated dose — verify with pharmacy or haematology
→ Consider using anti-Xa monitoring in obese patients (>150 kg)
