Haematology Oncology General Medicine Strong — standard oncology functional scale since 1949; correlates with survival

Karnofsky Performance Status Scale

Karnofsky Performance Status (KPS) scale rates functional capacity on a 100-point scale in 10-point increments. Used in oncology for treatment decisions, prognosis, and clinical trial eligibility.

Score interpretation

KPS 80–100 — Independent 80–100

Able to carry on normal activity with minor limitations. Corresponds to ECOG 0–1.

→ Eligible for standard chemotherapy. Most clinical trials require KPS ≥ 70. Treatment can proceed with standard doses.

KPS 50–70 — Some Assistance Required 50–70

Unable to work or carry on normal activity; requires some help. Corresponds to ECOG 2–3.

→ Careful assessment before intensive chemotherapy. Consider reduced-intensity regimens. MDT discussion. Palliative intent if KPS < 60.

KPS 0–40 — Unable to Care for Self 0–40

Requires significant support; hospitalisation likely. Corresponds to ECOG 3–4.

→ Palliative/supportive care focus. Active treatment rarely tolerated or beneficial. Comfort measures, symptom management, advance care planning.

Interpretation bands for the Karnofsky Score. Apply clinical judgement and local guidance.
