International Prognostic Score for Essential Thrombocythemia (IPSET) — thrombosis risk. Guides cytoreduction and aspirin decisions.
Score interpretation
→ Observation; aspirin 75mg if JAK2+ or cardiovascular risk factors; no cytoreduction required; 6-monthly haematology review
→ Aspirin 75-100mg daily; hydroxycarbamide if high platelet count; monitor for thrombosis/haemorrhage; cardiovascular risk modification; annual haematology
→ Cytoreduction mandatory: hydroxycarbamide first-line; pegylated interferon alfa for young patients or intolerance; aspirin 75mg daily; anticoagulation if prior VTE; target platelet <400; if HU resistant: anagrelide or ruxolitinib
