Kinetic eGFR (KeGFR) in AKI
Estimates GFR during acute kidney injury when creatinine is non-steady-state. More accurate than standard eGFR equations during AKI where creatinine is rising or falling.
Score interpretation
KeGFR <15 mL/min — kidney failure range during AKI
→ Nephrologist review; consider RRT; nephrotoxin avoidance; daily monitoring
KeGFR 15–30 mL/min — significant renal dysfunction
→ Close monitoring; optimise fluid status; drug dose adjustments; potential RRT threshold
KeGFR 31–60 mL/min — moderate dysfunction
→ Monitor recovery; review medication doses; maintain fluid balance; daily creatinine
KeGFR >60 mL/min — near-normal renal function during AKI recovery
→ Continue monitoring; restart nephrotoxins cautiously if clinically required
