Potassium Deficit Calculator

Estimates total body potassium deficit in hypokalaemia. Used to guide IV/oral potassium replacement.

Score interpretation

Normokalaemia 3.5–5

Serum potassium within normal range (3.5–5.0 mmol/L).

→ No replacement required. Reassess if symptomatic.

Mild Hypokalaemia 3–3.49

Mild hypokalaemia. Approximate deficit 100–200 mmol.

→ Oral potassium replacement (potassium chloride 40–80 mmol/day). Identify and treat cause. Repeat level in 24–48 hours.

Moderate Hypokalaemia 2.5–2.99

Moderate hypokalaemia. Estimated deficit 200–400 mmol.

→ Oral or IV potassium (IV rate ≤20 mmol/hour via central line; ≤10 mmol/hour peripherally). Cardiac monitoring if symptomatic. Co-replace magnesium.

Severe Hypokalaemia 0–2.49

Severe hypokalaemia (<2.5 mmol/L). Estimated deficit >400 mmol. Risk of arrhythmia.

→ Urgent IV replacement (cardiac monitored environment). IV K⁺ 20 mmol/hour via central line. Continuous ECG monitoring. Replace magnesium concurrently. Senior review.
