Predicts probability of testicular torsion to guide immediate surgery vs scrotal USS. High score = immediate exploration without USS delay.
Score interpretation
Low risk — USS appropriate
→ Scrotal USS to exclude torsion; monitor; early urology review
Intermediate risk — USS then surgery if positive
→ Urgent USS; if positive or equivocal: scrotal exploration without delay
High risk — immediate exploration
→ Emergency scrotal exploration without USS delay; bilateralorch idopexy if viable; orchidectomy if necrotic
