Cytomegalovirus immunoglobulin is a human plasma-derived antibody preparation rich in anti-CMV antibodies, used for prophylaxis of CMV disease in selected transplant recipients.

How it works
It provides passive immunity by supplying preformed antibodies that bind cytomegalovirus, supporting neutralisation and immune clearance during periods of risk.
Prescribing in practice
- Give by slow intravenous infusion with the rate titrated and the patient observed, as infusion-related and hypersensitivity reactions (including rare anaphylaxis, particularly in IgA-deficient recipients) can occur.
- As a blood-derived product it carries a theoretical risk of transmissible agents despite donor screening and viral-inactivation steps; batch records should be retained.
- It may transiently interfere with the response to live virus vaccines, which should be appropriately spaced.
Monitoring
Monitor infusion-site tolerance, vital signs and for hypersensitivity during and after administration.
Counselling the patient
- Tell the team at once if you feel flushed, wheezy, itchy or unwell during the infusion.
- This is a human plasma product given to help prevent CMV infection after transplant.
