Score interpretation

Fit 0

→ Routine care. Health promotion. Reassess at annual review.

Mild frailty 1

→ Annual GP frailty review. Address polypharmacy (STOPP/START). Falls and bone-health review (FRAX). Encourage strength-balance exercise.

Moderate frailty 2

→ CGA per BGS Toolkit. Care-coordination via multidisciplinary frailty team. Anticipatory care plan / ReSPECT. Carer assessment.

Severe frailty 3

→ Per NHS England Enhanced Service: medication review, GP-led CGA, advance care planning. Consider community matron. Discuss preferred place of care; hospice referral if life-limiting trajectory. Avoid hospital admission where possible.
