Rai Staging System for CLL

Staging system for chronic lymphocytic leukaemia (CLL) based on clinical and haematological features. Predicts prognosis and guides treatment.

Score interpretation

Stage 0 — Low Risk 0

Low risk. Median survival >10 years.

→ Watch and wait; no immediate treatment indicated. Monitor with regular FBC and clinical examination.

Stage I–II — Intermediate Risk 1–2

Intermediate risk. Median survival 7–9 years.

→ Watch and wait unless active disease criteria met. Consider enrolment in clinical trials.

Stage III–IV — High Risk 3–4

High risk. Median survival 1.5–5 years.

→ Treatment indicated if symptomatic. Consider chemoimmunotherapy (CLL14, GAIA) or targeted therapy (BTK/BCL2 inhibitors). Haematology review.
