Prognostic Indicator Guidance (PIG) / Gold Standards Framework

Identifies patients in the last year of life using Gold Standards Framework prognostic indicators. Triggers palliative care registration and ACP.

Score interpretation

Unlikely last year of life

→ Continue routine care; reassess at next review; document baseline functional status

Possible last year of life — consider palliative register

→ Initiate advance care planning conversation; palliative care team referral; review medications; carer assessment; GSF register if agreed

High likelihood last year of life

→ Register on GSF/palliative care register; anticipatory prescribing; DNACPR discussion; preferred place of care/death; specialist palliative care referral; family meeting

Interpretation bands for the GSF Prognostic Indicators. Apply clinical judgement and local guidance.
