Birmingham Vasculitis Activity Score (BVAS)
Measures disease activity in ANCA-associated vasculitis (GPA, MPA, EGPA) across nine organ systems. Guides immunosuppressive therapy.
Score interpretation
Low activity or remission
→ Low disease burden; maintenance immunosuppression; regular monitoring
Moderate active vasculitis
→ Systemic steroids; consider rituximab or cyclophosphamide; organ-specific management
Severe active vasculitis
→ High-dose IV methylprednisolone; rituximab or IV cyclophosphamide; plasma exchange if severe renal disease; monitor for PCP prophylaxis need
Interpretation bands for the BVAS. Apply clinical judgement and local guidance.
