PSA Density = PSA / Prostate Volume (from TRUS). Helps differentiate benign BPH from prostate cancer.

Score interpretation

Low Density — BPH Likely 0–0.09

PSA density <0.10 ng/mL/mL: More likely benign BPH

→ Continued surveillance; PSA monitoring; no immediate biopsy required if other features benign

Borderline — Consider Biopsy 0.1–0.14

PSA density 0.10–0.14: Borderline zone

→ MRI prostate first (mpMRI); biopsy based on clinical picture and MRI result

High Density — Cancer Suspected ≥ 0.15

PSA density ≥0.15 ng/mL/mL: Increased risk of prostate cancer

→ Urgent mpMRI; urology referral; TRUS-guided or fusion biopsy recommended
