PSA Velocity and PSA Density

Calculates PSA velocity (rate of PSA rise per year) and PSA density. PSA velocity >0.75 ng/mL/year or PSA density >0.15 ng/mL/mL suggests cancer risk.

Score interpretation

PSA velocity < 0.75 ng/mL/yr — Low cancer risk

→ Reassurance; continue PSA surveillance per EAU/NICE guidelines

PSA velocity 0.75-2 ng/mL/yr — Elevated risk

→ MRI prostate (mpMRI); if PIRADS 3+: targeted biopsy; urology referral

PSA velocity > 2 ng/mL/yr — High cancer risk

→ Urgent mpMRI; prostate biopsy if not already done; staging CT and bone scan if biopsy positive
