Ropeginterferon alfa-2b is a long-acting, mono-pegylated recombinant interferon alfa used as a specialist cytoreductive treatment for polycythaemia vera.

How it works

It binds type I interferon receptors, activating JAK-STAT signalling that suppresses clonal myeloproliferation in the bone marrow, helping to control red-cell and platelet production.

Prescribing in practice

  • Contraindicated in significant pre-existing psychiatric disease (notably severe depression or suicidal ideation), autoimmune disease, and decompensated liver disease, given the risk of neuropsychiatric and autoimmune reactions.
  • Initiated and titrated only under haematology supervision, with dose individualised against haematocrit, platelet, and white-cell response.
  • Use cautiously with thyroid disorders and monitor for flu-like symptoms, which are common particularly during initial therapy.

Monitoring

Monitor full blood count, liver function, thyroid function, blood glucose, and mental state regularly throughout treatment.

Counselling the patient

  • Report low mood, anxiety, or thoughts of self-harm promptly, as interferons can affect mood.
  • Flu-like symptoms are common early on and often settle; taking the injection in the evening may help.
  • Effective contraception is advised during treatment.
