ARC-HBR Criteria for High Bleeding Risk in PCI

Academic Research Consortium for High Bleeding Risk (ARC-HBR) criteria. HBR = 1 major OR ≥ 2 minor criteria. Guides DAPT duration post-PCI.

Score interpretation

Standard Bleeding Risk 0–9

Fewer than 1 major or 2 minor ARC-HBR criteria. Not classified as High Bleeding Risk.

→ Standard DAPT duration post-PCI (12 months for ACS; 6 months for stable CAD). Routine anticoagulation as indicated.

High Bleeding Risk (ARC-HBR Positive) 10–200

1 major OR ≥ 2 minor criteria met. Classified as High Bleeding Risk.

→ Consider abbreviated DAPT (1–3 months post-PCI). Use thin-strut DES. Haematology/pharmacy review. Minimise PPI need.
