PRECISE-DAPT Score for Bleeding on DAPT

Predicts out-of-hospital bleeding risk during dual antiplatelet therapy (DAPT) to guide DAPT duration decisions post-PCI.

Calculate using Cockcroft-Gault equation

Score interpretation

Low Bleeding Risk 0–24

PRECISE-DAPT < 25: Low out-of-hospital bleeding risk. Extended DAPT (24 months) favoured if ischaemic risk justifies.

→ Extended DAPT (24 months post-PCI) is reasonable. Reassess at 12 months.

High Bleeding Risk 25–100

PRECISE-DAPT ≥ 25: High bleeding risk. Shorter DAPT (3–6 months) preferred.

→ Short-course DAPT (3–6 months). Then aspirin monotherapy. Balance against ischaemic risk.

Interpretation bands for the PRECISE-DAPT. Apply clinical judgement and local guidance.
