Wound Healing Assessment (PUSH Tool)

Pressure Ulcer Scale for Healing (PUSH) assesses wound size, exudate, and tissue type to monitor healing progress. Used for chronic wounds including pressure ulcers and venous leg ulcers.

Score interpretation

Healing well

→ Continue current dressing regime; weekly monitoring; document progress; address nutritional status

Partial healing — monitor closely

→ Review dressing choice; tissue viability referral; ensure adequate nutrition and offloading; consider compression for venous wounds; check for infection

Poor healing or worsening

→ Urgent tissue viability nurse; debridement if necrotic tissue; wound swab if infection signs; vascular surgery review; consider NPWT (vacuum); MDT wound clinic
