How it works

It binds tubulin and inhibits microtubule assembly, halting cell division at metaphase.

Prescribing in practice

  • It is for intravenous use only and is fatal if given intrathecally; spinal-injection safety procedures must be followed.
  • Myelosuppression and peripheral neuropathy are the main dose-limiting toxicities.
  • It is a vesicant specialist cytotoxic requiring extravasation precautions and expert oversight.

Monitoring

Monitor full blood count before dosing and assess regularly for peripheral neuropathy and signs of infection.

Counselling the patient

  • Report signs of infection such as fever or sore throat without delay.
  • Report numbness, tingling or muscle weakness.
  • Alert staff immediately to any pain or swelling at the injection site.

Evidence & guidelines

Its use is supported by established chemotherapy practice within specialist oncology services.
