AHA/ACC 2023 PREVENT equations predict 10- and 30-year risk of total cardiovascular disease (CVD) events including heart failure. Replaces pooled cohort equations. Validated across diverse populations.

Low 10-Year CVD Risk (<5%) 0–5

10-year CVD risk <5%. Low risk population.

→ Lifestyle optimisation. Primary prevention counselling. Reassess in 4-6 years. Statins generally not indicated.

Borderline Risk (5-9.9%) 5–10

10-year CVD risk 5-9.9%. Borderline risk.

→ Risk discussion with shared decision-making. Consider risk-enhancing factors. Moderate-intensity statin if risk-enhancing factors present.

Intermediate Risk (10-19.9%) 10–20

10-year CVD risk 10-19.9%. Intermediate risk.

→ Statin therapy recommended. LDL target < 2.6 mmol/L. Lifestyle intensification. Consider CAC score to guide further.

High Risk (>=20%) 20–100

10-year CVD risk >=20%. High risk.

→ High-intensity statin. LDL target < 1.8 mmol/L. Aspirin consideration. Aggressive risk factor modification. Cardiology referral.
