SCORE2 — 10-Year CVD Risk (Age 40–69)

European Heart Score 2 — estimates 10-year risk of fatal and non-fatal cardiovascular events for adults aged 40–69 without known CVD or diabetes.

Used in: Hypertension

Score interpretation

Low 10-Year CVD Risk 0–4

SCORE2 < 5%: Low risk of fatal or non-fatal CVD event over 10 years.

→ Lifestyle advice. Repeat risk assessment in 5 years. No statin indicated solely on risk level.

Moderate 10-Year CVD Risk 5–9

SCORE2 5–9%: Moderate risk. Individualised discussion about statin therapy.

→ Lifestyle modification. Consider statin if LDL-C elevated despite lifestyle change.

High 10-Year CVD Risk 10–14

SCORE2 10–14%: High risk. Statin therapy recommended.

→ Statin therapy. BP treatment if elevated. Lifestyle modification intensification.

Very High 10-Year CVD Risk 15–100

SCORE2 ≥ 15%: Very high risk. Aggressive lipid-lowering and BP management.

→ High-intensity statin. LDL-C target < 1.4 mmol/L. BP target < 130/80. All cardiovascular medications as indicated.
