Rai Staging System for CLL

Rai staging system for Chronic Lymphocytic Leukaemia (CLL) classifying disease into stages 0–IV based on lymphocytosis, organomegaly, and cytopenias.

Score interpretation

Stage IV — High Risk

→ Rai Stage IV: Thrombocytopenia present. High-risk CLL. Median survival ~19 months (historical). Refer for treatment consideration.

Stage III — High Risk

→ Rai Stage III: Anaemia present. High-risk CLL. Median survival ~19 months (historical). Early treatment initiation usually indicated.

Stage II — Intermediate Risk

→ Rai Stage II: Splenomegaly/hepatomegaly. Intermediate-risk CLL. Median survival ~7 years. Treatment when symptomatic or IWCLL criteria met.

Stage I — Intermediate Risk

→ Rai Stage I: Lymphadenopathy only. Intermediate-risk CLL. Median survival ~9 years. Watch and wait unless symptomatic.

Stage 0 — Low Risk

→ Rai Stage 0: Lymphocytosis only. Low-risk CLL. Median survival ~>10 years. Watch and wait strategy; no treatment needed.
