Low risk (5-y OS ~83%) 0

→ Standard R-CHOP × 6.

Low-intermediate risk (~69%) 1

→ R-CHOP × 6 with attention to dose intensity and PET response.

High-intermediate risk (~46%) 2

→ Consider intensified therapy or trial enrolment; CNS-IPI assessment.

High risk (~32%) 3

→ Trial enrolment / intensified regimens; CNS prophylaxis indicated; close interim assessment for early salvage.
